User Access Request

This form allows you to request new user roles to access this system. Your submission will be followed up by email from the system administrator.
Please supply following information:
  • User First, Middle and Last name
  • The requested role(s) type: primary, delegate (always), delegate when primary is off, or view only
  • The requested role(s): Initiator, Department Admin or Department Chair
  • For which department and sub-department
  • Justification for request
